At Compass Memorial Healthcare, our Social Services team is dedicated to supporting patients and their families by assessing emotional, social, and healthcare-related needs and connecting them with resources that best meet those needs. We are here to help you navigate care decisions, access community services, and feel supported throughout your healthcare experience. 

Our Services Include:

  • Comprehensive discharge planning
  • Education, information, and referrals to programs and services
  • Advocacy and guidance through healthcare decisions
  • Arranging in-home services such as home health, meals, or medical equipment
  • Assistance with insurance or financial concerns
  • Support with long-term care placement decisions
  • Guidance in completing Advance Directives (Living Will, Healthcare Power of Attorney, IPOST)
  • Coordination of skilled swing-bed, Transitional Care and hospice care
  • Answering questions from the public about hospital programs and providing referrals as needed
